Agreement to Terms

By accessing or using Lighthouse Health's platform and services, you agree to be bound by these Terms of Service and our Privacy Policy. If you do not agree to these terms, please do not use our services.

Description of Services

Lighthouse Health provides AI-powered revenue recovery analytics for Federally Qualified Health Centers (FQHCs) and healthcare organizations, including:

  • Provider performance tracking and chart closure analytics
  • No-show pattern analysis and reduction strategies
  • Claim denial management and recovery recommendations
  • Payer contract tracking and renewal management
  • Provider credentialing monitoring and alerts
  • AI-powered recommendations for revenue optimization

Subscription and Payment

Subscription Plans

We offer annual subscription plans (Starter, Professional, and Enterprise) with pricing based on the number of providers, locations, and features selected. All pricing is provided during the sales process and confirmed in your subscription agreement.

Payment Terms

  • Annual subscriptions are billed in advance
  • Payment is due within 30 days of invoice
  • Late payments may result in service suspension
  • Pricing is locked for the subscription term (typically 1 year)
  • Early adopter rates are locked for 3 years as specified in your agreement

Refunds

We offer a 30-day satisfaction guarantee. If you're not satisfied within the first 30 days, we'll provide a full refund. After 30 days, subscriptions are non-refundable but may be canceled with 30 days' notice.

User Responsibilities

Account Security

You are responsible for:

  • Maintaining the confidentiality of your login credentials
  • All activities that occur under your account
  • Notifying us immediately of any unauthorized access
  • Using strong passwords and enabling multi-factor authentication

Acceptable Use

You agree not to:

  • Use the platform for any unlawful purpose
  • Attempt to gain unauthorized access to our systems
  • Reverse engineer, decompile, or disassemble our software
  • Share your account with unauthorized users
  • Upload malicious code or viruses
  • Scrape or harvest data without permission
  • Resell or redistribute our services

Data Accuracy

You are responsible for the accuracy and completeness of data you upload to our platform. We provide analytics based on the data you provide; garbage in, garbage out.

Intellectual Property

Our IP

Lighthouse Health owns all rights to our platform, software, algorithms, AI models, documentation, and trademarks. Your subscription grants you a limited, non-exclusive, non-transferable license to use our services during your subscription term.

Your Data

You retain all ownership rights to your data. We have a limited license to use your data solely to provide our services and improve our platform (in aggregated, de-identified form).

Service Level and Support

Availability

We target 99.5% uptime (excluding scheduled maintenance). We will notify you of planned maintenance at least 48 hours in advance when possible.

Support

Support levels vary by plan: Starter (email, 24hr response), Professional (email/chat, 4hr response, account manager), Enterprise (24/7, 1hr response, dedicated specialist).

No Medical Advice

Lighthouse Health provides operational and financial analytics only. We do not provide medical advice, diagnosis, or treatment. All clinical decisions remain your responsibility.

AI Recommendations

Our AI-powered recommendations are based on data analysis and industry best practices. They are suggestions only and should be reviewed by your team before implementation. We do not guarantee specific revenue recovery results.

Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, LIGHTHOUSE HEALTH'S TOTAL LIABILITY SHALL NOT EXCEED THE AMOUNT YOU PAID IN THE 12 MONTHS PRECEDING THE CLAIM. WE ARE NOT LIABLE FOR INDIRECT, INCIDENTAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES.

Term and Termination

Subscription Term

Subscriptions are annual and automatically renew unless canceled with 30 days' written notice before the renewal date.

Termination by You

You may cancel your subscription at any time with 30 days' written notice. No refunds for unused time (except during the 30-day satisfaction guarantee period).

Termination by Us

We may suspend or terminate your account for: (a) non-payment, (b) violation of these terms, (c) illegal or harmful conduct, or (d) with 30 days' notice for any reason.

Effect of Termination

Upon termination, your access ends immediately. We will provide a final data export in CSV format. Your data will be deleted after 90 days unless legally required to retain it.

Changes to Terms

We may modify these Terms at any time. Material changes will be communicated via email 30 days in advance. Continued use after changes constitutes acceptance. If you don't accept changes, you may terminate your subscription within the 30-day notice period.

Governing Law and Disputes

These Terms are governed by the laws of the State of Georgia, USA, without regard to conflict of law provisions. Any disputes will be resolved through binding arbitration in Atlanta, Georgia, except for intellectual property claims.

Miscellaneous

  • Entire Agreement: These Terms, your subscription agreement, and our Privacy Policy constitute the entire agreement
  • Assignment: You may not assign your rights; we may assign ours with notice
  • Severability: If any provision is unenforceable, the remainder remains in effect
  • No Waiver: Our failure to enforce a right does not waive that right
